『ウェビナー報告日誌 2020「Rural Skills」編 vol.6 ―「相手の立場」から考える―』

 

離島やへき地で医師として闘う上で助けとなる手技や知識を中心に、様々な分野で活躍される経験豊富な専門医の先生方から実践的なレクチャーを受ける『Rural Skills』。

 

昨年四月から始まった四つの月例ウェビナーすべての最後を飾ることとなった今回は、ゲネプロ指導医の山口 卓哉先生が壇上に登り、山口先生が「医師として深く考えさせられた症例」を題材に、研修生たちに貴重な体験談と共有しておきたい “学び” が伝えられました。

 

 

 

 

症例:人生初のがん検診を受けた男性

 

 

【患者について】

 

 

  ・70歳の男性

 

  ・高血圧と糖尿病で近医外来に通院中

 

  ・これまで入院歴なし。元気

 

  ・がん検診は受診歴なし

   

   ⇒ 家族からのすすめを受け、初めてのがん検診を受診

 

 

 

【 検診・施術の結果 】

 

 

  ・初めてのがん検診で大腸ポリープを発見

  

  ・ESD後の合併症で人工肛門増設

 

  ・人工肛門閉鎖の合併症で吻合部出血

 

 

 

 

今回の講義においては、実際に山口先生のご家族の方が経験され、また先生ご自身もその顛末を非常に近い距離で見届けた症例が取り上げられることとなりました。

 

普段は「医師」という「診る」側の立場から、「患者とその家族」という「診られる」側へとその立場が一変したことで、非常に多くの葛藤や悩み、不安や怒りなどを経験し、その中で人としても医師としても非常に多くの気づきや学びを得ることとなったそうです。

 

 

研修生の先生方にも、「いつでも自分が診られる側に立つ可能性があることを、一人の医師として常に忘れないようにしてほしい」と訴え、自分自身の立場からだけでなく、様々な立場からの目線に立って物事を見ることの重要性を強調されました。

例えば、今回の症例だけをとっても、これだけの異なる立場が存在しており、それだけ異なる数の事情や理屈、考え方などがせめぎ合っているということを理解しておくことが大事だと語る山口先生。

 

実際、研修生の先生方からも、自分とは異なる立場にあったり物の見方をしたりする同僚や患者との関係の中で葛藤や悩みを抱えた経験が共有されましたが、皆さん同じような経験があったのか、一様に身につまされながら耳を傾けていたようでした。

 

 

 

そして今回、講義の最後では、

 

 

 

  1.医師としての限界を知る

 

  2.医師がコントロールできることなんて僅かしかない

 

  3.医療者と患者(家族)の認識には乖離がある

 

  4.身近で大切な人には検診を勧めてほしい

 

 

 

というメッセージが、これまでの研修への労いと今後に向けた激励の言葉とともに研修生の先生方に贈られ、今回の講義が、そして一年間のウェビナーが無事に締め括られることとなりました。

 

 

 

 

 

 

 

The Rural Skills, a webinar that focuses on improvement and development of the registrar’s skills required for a GP in rural and remote contexts, was conducted the other day.

 

Actually, that was the final of not only the Rural Skills itself, but also the whole of the regular monthly webinars. Dr. Takuya Yamaguchi, the attending doctor in Genepro, came to the stage and shared his memorable experiences that impacted his mindset as a doctor in a big way with registrars.

 

 

According to him, the case history he introduced in the lecture was the one which his father had actually experienced and he also had witnessed everything from beginning to end up close.

 

Usually, doctors tend to loss sight of the fact that there is always a possibility that he/she may be suddenly forced them into position of a patient too. He had experienced significant conflict between himself as a doctor and one as a son of the patient through the case, Dr. Yamaguchi said.

 

 

Also, he stressed the importance of having the attitude that endeavors to see something in the same way that patients does.

 

Through the case, he had paused to realize that there existed various values or mindsets even in a case and had had a renewed sense of the importance of putting himself in other people’s shoes, according to him.

 

 

At the very end of the session, Dr. Yamaguchi provided the registrars with encouragement in their future with the words of appreciation for their sincere participation in all of webinars through a year.

 

The webinar, finally, was dropped a curtain without any mishap.
