研修医の先生方が、実際に研修先の病院で遭遇した症例や体験した得難い教訓などを題材に、仲間の研修生たちと情報共有や症例検討を行うためのウェビナー『Registrar’s Lecture』。

 

今回は、千葉県匝瑳市にある匝瑳市民病院にて研修中の蛯子 隼先生と高知県宿毛市にある大井田病院で研修中の西場 大喜先生のお二人が発表者となり、それぞれ「匝瑳市民病院の特徴」と「エコーの使い方」をテーマに発表していただきました。

 

 

 

 

発表の前半部では、蛯子先生の研修先病院である匝瑳市民病院の特徴に関する紹介が行われ、同病院にはどういった魅力や強みなどがあり、どのような環境の中で日々の診療や研修が行われているのかについて教えていただいたほか、研修中に印象に残った症例などについても情報が共有されることとなりました。

 

一方、後半部を担当された西場先生の発表は、研修開始から半年間にわたり精力的に習得に取り組んできた「リニアエコープローブ」に関するものとなり、先生自身が体得された使用上のコツや注意点などを交えつつ、印象に残った症例についても情報を共有していただきました。

 

 

 

今でこそすっかり環境や慣習に馴染み、のびのびと楽しみながら研修の日々を送れているものの、研修が開始された当初は、それまで勤務していた病院と匝瑳市民病院との大きな “違い” に戸惑うことも多々あったと語る蛯子先生。

 

そういった “違い” に適応していく中で、一人の「医師」としてこれまでの自分になかった物の見方や考え方ができるようになったそうですが、中でも「患者さん側の利益と病院側の利益とのバランスを考えられるようになった」ことで、「病院の状況（事情）に合わせて、治療を柔軟に選べるようになった」ことは、最も大きな変化の一つとなったようでした。

 

 

 

そして、今年4月より、エコーの習得に意欲を燃やす若手医師を対象とした「奨学エコー制度」を利用し、「運動器エコー」の手技と知識に磨きをかけていた西場先生でしたが、今回の発表では、実際に研修中に西場先生が経験した症例を参考にしながら、およそ半年間におよぶ ”修行” の成果が、他の研修生たちに共有されました。

 

 

また、研修に参加してからというもの、それまで専攻していた「麻酔科」を足がかりとして、「皮膚科」や「整形外科」といった “専門外の分野” にも医師としての裾野を拡げる努力を続けてきたという西場先生。

 

発表の終盤には、そんな先生から他の研修生方に対して「専門科以外の診療を経験し、新たに気付かされたことはありますか？」という質問が投げかけられることとなりましたが、やはりそこは同様の努力と苦労を共有する仲間同士、交わされる意見や体験談に誰もがそれぞれに共感しながら耳を傾けている姿が印象的でした。

 

 

 

 

 

 

 

The Registrar’s Lecture, a webinar for enhancing the registrars’ abilities and the quality of their training through sharing their experiences and knowledges among their fellows, successfully dropped a curtain, the other day.

 

Actually ,this time, Dr. Jun Ebiko and Dr. Taiki Nishiba played roles as presenters and they talked about “memorable cases” and “echography “, respectively.

 

 

Firstly, Dr. Ebiko talked about the Sosa Municipal Hospital, the hospital that she had been working from this April, and introduced its advantages as a hospital to other registrars. Then, she referred to some shocking differences between the current hospital and the former hospital at which she had worked.

 

According to her, although she had gotten a cultural shock and had been perplexed at first, she had endeavored to adapt to the new environment and finally acquired the ways of thinking that she had not had before through the adaptation process.

 

“Now I can achieve a good balance between the patients’ benefits and the hospital’s. It enables me to offer appropriate treatments to patients flexibly depending on the various conditions of the hospital”, said Dr. Ebiko. It seemed that she have been sure her development as a doctor.

 

 

 

In the latter part of the session, Dr. Nishiba lectured about echography with linear echo probe. Actually, he had learned earnestly about echography from this April, so he shared the tips and perception that he eventually obtained after six-months strenuous efforts with other registrars.

 

 

Also, “I had currently been struggling to extend the scope of practice as a doctor”, Dr. Nishiba stressed in his speech.

 

Other registrars empathized with his attitude, then they began to share their experiences with and give some advices to each other. Fortunately enough, everyone finally succeeded in contribute to mutual growth with respect to one another.
