『ウェビナー報告日誌 2023「G’day Mate」編 vol.4 ― 症例ディスカッション ―』
「海外の医療現場でも通用する実践的な英語力」の獲得を目指してオーストラリア人のネイティブ講師、ジャスミン先生と英会話力を鍛えるためのウェビナー『G’day Mate』。
今回のセッションでは基本に立ち返り、実践に即したロールプレイング形式でのレッスンを通じて英語を話す力が鍛えられることとなりました。
いつものように研修生同士で二人一組となり、「医師」と「患者」とに役を分けての即興英会話劇。
純粋に「鑑別や対応の難しい症状」を訴える”患者”もいれば、「意表を突くような意外な症状」を訴える“患者”まで様々なのもいつも通り。
しかしながら、既に半年以上にわたりレッスンを重ねてきたこともあり“医師”も随分と慣れたもので、ロナルド先生から教わった診察の型や「ICE」を駆使しながら的確に質問やコミュニケーションを図りつつ、必要な情報を着実に“患者”から引き出していく姿が印象的でした。

＝患者の受診理由を適切に尋ね、正しく理解する上で重要となる質問モデル
１．Idea（解釈モデル）
「何か原因がありそうですか？」
☆患者自身の「現在の症状・状態に対する理解・解釈」を把握する
病気や身体のことに関して、とても不思議な解釈をしている場合や、文化的な背景に基づいた考えを持っている場合などもあるため、「患者自身の理解・解釈」について質問することは有効。
２．Concerns（気になっていること）
「特に何が気になっていますか？」
☆「患者の抱いている不安や心配」について探る
時に、医師が想像もしないことを気にしている場合などもあるため、その「気になっていること」に対応・対処しない限り、患者からの満足を得ることは難しい。
３．Expectations（診察に期待していること）
「今日の診察で何をしてもらいたいですか？」
☆「患者が真に何を求めているのか」を正しく理解する
「医師が考えもしなかった理由」や「非常に単純な理由」などで受診する場合もあるため、症状や病気だけに重点を置いた診療では、患者の期待に応えることが難しくなってしまう点に要注意。
また今回、レッスンの最後には「次回のレッスンで扱いたいトピックやテーマ」についてジャスミン先生から研修生へと意見を求める一幕も。
最終的に「各自それぞれの『外来でのコツ』を1つずつ持ち寄って発表・共有し合う」というアイデアに賛同が集まり、研修生方の自主性と積極性が発揮される形で次回のテーマが決まることとなりました。
本当に早いもので、次回は年内最後のレッスン。クリスマスシーズンらしく、研修生同士で素敵な“プレゼント交換”が行われることとなりそうです。
Again this month, we fortunately held the webinar for improving the registrars’ English abilities: the G’day Mate.
Throughout the session, the registrars remembered the basics and conducted a role-playing. In other words, they all were divided into a pair, then again divided into two roles: doctors and patients.
Anyway, as is often the case with the role-playing session in the G’day Mate, the “patients” tend to voice confusing or complicated complaints out of their slight mischief.
“Doctors” had also accumulated lots of experiences through the sessions since this webinar had begun, however, so they properly and serenely extracted information from the “patients” by utilizing the model of diagnosis previously lectured by Dr. McCoy.
To our great delight, it seemed that not only their English skills but also positiveness toward English itself has consistently been developed through the lessons.
Moreover, in terms of the registrars’ positiveness, they appealed to Ms. Millman that conduct a session for mutually sharing their original tips in medical examination, in the end of the lecture.
So, their request would be complied at the next session, which is to be held in the Christmas season. And, it should be a wonderful present for them each other.
